This form helps you to think about your life and legacy. We will support your family to accommodate your wishes, while meeting their needs during their time of grief.

Celebrating Your Life

Your answers will give direction to family, friends, and IUC of how your funeral will honour you.

Your Legacy of Generosity

When you die, we notify the community with an email including your obituary, which is then posted on our website. People are invited to honour you by making a donation to IUC and the organizations you name in your obituary. You can strengthen a ministry you love by designating you gift using the list below.
